Complete the online form below to register and order our products. We will send you a confirmation email once we receive your order.

We will ship your Macula Risk / Vita Risk test kits to your office. This includes five test kits with complete instructions, prepaid shipping envelopes, plus patient and doctor reference materials.

Expect an average of three to five weeks for delivery.

Registration is currently limited to North American (US & Canadian) clinics only.
